Subject Access Request (SAR) Form (Referred to in the above Privacy Notice)

You are entitled to submit subject access requests all year round, but please bear in mind that it may be necessary for us to extend the response period when requests are submitted over holiday periods. This is in accordance with article 12(3) of the GDPR, and will be the case where the request is complex – for example, where we need multiple staff to collect the data or external stakeholders.

Please use this SAR Form or e-mail info@st-marks-southampton.org.uk to lodge a request and include full details of what you are requesting and from which source. The form can be found below. Please also include details of how you would like to receive your response (i.e. via E-mail or in person).
